Behçet Hastalarında Tam Kan Sayımı ve Biyokimyasal Parametrelerin Değerlendirilmesi

ÖZET
Amaç: Behçet hastalarının çeşitli kan parametrelerini değerlendirerek
sağlıklı kontroller ile karşılaştırılması amaçlanmıştır. Gereç ve
Yöntemler: Behçet hastalığı tanısı olan 50 (30 kadın, 20 erkek) hasta ve
46 (27 kadın, 19 erkek) sağlıklı gönüllü çalışmaya dâhil edildi. Demografik
veriler kaydedildi. Behçet hastalarının hastalık süresi ve hastalık
şiddeti Behçet Hastalık Klinik Aktivite Form (BHKAF) indeksi ile değerlendirildi.
Hasta ve kontrol grubunun tam kan sayımı, vitamin B12,
demir, demir bağlama kapasitesi, ferritin, folat, eritrosit sedimentasyon
hızı [erythrocyte sedimentation rate (ESR)], C-reaktif protein (CRP), kan
üre azotu [blood urea nitrogen (BUN)], kreatinin, alanin transaminaz
(ALT), aspartat transaminaz (AST) gibi tam kan sayımı ve bazı biyokimyasal
parametreleri karşılaştırıldı. Bulgular: Hasta ve kontrol gruplar
arasında yaş ve cinsiyet açısından anlamlı fark bulunmadı (p>0,05).
Çalışmaya alınan Behçet hastalarının hastalık süresi 12,76±10,14 yıldı.
BHKAF ile değerlendirilen hastalık aktivite indeksi 1,44±0,25 olarak hesaplandı.
Hasta ve kontrol grubu arasında tam kan sayımı parametrelerinden
hemoglobin, hematokrit, beyaz küre, trombosit, nötrofil sayısı,
lenfosit sayısı, nötrofil/lenfosit oranı, platelet/lenfosit oranı, ortalama korpüsküler
hacim, kırmızı kan hücresi dağılım genişliği, ortalama trombosit
hacmi (MPV) değerleri ile vitamin B12, demir, demir bağlama
kapasitesi, ferritin, folat, ESR, BUN, kreatinin, ALT, AST değerleri arasında
istatistiksel olarak anlamlı fark bulunmadı (p>0,05). CRP düzeyi
Behçet hastalarında istatistiksel olarak anlamlı derecede yüksek tespit
edildi (p˂0,001). Sonuç: Hasta grubunda kontrol grubuna göre CRP düzeyinin
yüksek bulunması, Behçet hastalığının inflamatuar bir süreç olması
ile ilişkilidir. Diğer tam kan sayımı ve biyokimyasal parametrelerde
fark bulunmaması, çalışmaya alınan Behçet hastalarının hastalık aktivite
indeksi değerlerinin düşük olması ile ilişkili olabilir.

ABSTRACT
Objective: The aim of this study was to evaluate various
blood parameters in patients with Behçet’s disease and compare them
with healthy controls. Material and Methods: Fifty (30 females, 20
males) patients and 46 (27 females, 19 males) healthy volunteers with
Behçet’s disease were included in the study. Demographic data were
recorded. Disease duration and disease severity of Behçet’s disease patients
were evaluated with Behçet Disease Clinical Activity Form
(BDCAF) index. biochemical parameters such as vitamin B12, iron,
iron-binding capacity, ferritin, folate, sedimentation [erythrocyte sedimentation
rate (ESR)], C-reactive protein (CRP), blood urea nitrogen
(BUN), creatinine, alanine transaminase (ALT), aspartate transaminase
(AST) and complete blood count parameters were compared between
patients and controls. Results: There was no significant difference between
patient and control groups in terms of age and gender (p>0.05).
The disease duration of Behçet’s patients included in the study was
12.76±10.14 years. Disease activity index evaluated by BDCAF was
calculated as 1.44±0.25. Hemoglobin, hematocrit, white blood cell,
platelet, neutrophil count, lymphocyte count, neutrophil/lymphocyte
ratio, platelet/lymphocyte ratio, mean corpuscular volume (MCV), red
blood cell distribution width (RDW), mean platelet volume (MPV) values
and vitamin B12, iron, iron-binding capacity, ferritin, folate, ESR,
BUN, creatinine, ALT, AST values were not statistically significant
(p>0.05). CRP levels were significantly higher in Behçet’s patients
(p˂0.001). Conclusion: The higher CRP level in the patient group compared
to the control group is related to the fact that Behçet’s disease is
an inflammatory process. The absence of any difference in other blood
counts and biochemical parameters may be related to the low disease
activity index values of the patients with Behçet’s disease.
