Peroneal nöropati, mononöropatiler içinde %15 oranında gözlenirken bariatrik cerrahi sonrası nadir gelişen bir komplikasyondur. Etyolojisi multifaktöriyel olmakla birlikte, hızlı kilo kaybına bağlı kompresyon ve nütrisyonel imbalans en sık suçlanan faktörlerdir. Peroneal sinir fibula başında sadece cilt ve cilt altı doku ile kaplı olduğundan hızlı kilo kaybı ile subkütan dokunun kaybı peroneal siniri fibula başında travmaya hassas hâle getirmektedir. Bu çalışmada, obezite cerrahisi sonrası altı ayda 34 kg kaybeden ve düşük ayak tablosu nedeni ile kliniğimize başvuran olgunun sunulması amaçlanmıştır. Yapılan fizik muayenesi ve elektrofizyolojik incelemelerinde peroneal nöropati olduğu saptanan olgu, nütrisyonel destek ve fizik tedavi programı ile izlem altına alınmış, dördüncü ayda şikâyetleri gerilemiştir. Peroneal nöropatinin artan obezite cerrahileri nedeni ile hızlı kilo kaybına sekonder gelişebileceği, optimal diyet düzenlemesi ve nütrisyonel destek sağlanması ile en aza indirilebileceği akılda tutulmalıdır.

Peroneal neuropathy is rare complication after bariatric surgery although it is observed in 15% mononeuropathy. The etiology of peroneal neuropathy is multifactorial and most often accused of these factors that due to rapid weight loss and nutritional imbalance. Even though peroneal nerve was covered with only skin and subcutaneous tissue along fibular head and the loss of subcutaneous tissue due to rapid weight loss makes it sensitive to the trauma in fibular head. In this case, the patient who losing weight 34 kg in 6 months after surgery for obesity and suffer from foot drop is presented. Result of the physical examination and electrophysiological studies, peroneal neuropathy was obtained and the patient have been followed by physical therapy and nutritional support program, it decreased complaints in 4th months. Due to increasing obesity surgery of peroneal neuropathy can develop secondary to rapid weight loss, it should be considered at least to be reduced by ensuring optimal dietary modification and nutritional support.
