Ankilozan Spondilit Tanılı Hastaların Kardiyovasküler Hastalıklar Risk Faktörleri Bilgi Düzeylerinin Değerlendirilmesi

ÖZET
Amaç: Kardiyovasküler hastalıklar (KVH); inme, periferik damar
hastalıkları ve koroner arter hastalıklarını içeren bir grubu tanımlar. Ankilozan
spondilit (AS) hastalarında, KVH açısından artmış risk mevcuttur. AS
hastalarının ölüm oranları genel popülasyonun 1.5 katıdır. Bu hastalarda
mortalitenin %20-40'ından kardiyovasküler olaylar sorumludur. Bu riskin
tespiti ve azaltılması için çeşitli araçlar ve yöntemler araştırılmaktadır. Bu
çalışmada, araştırmalara temel olması amacıyla AS hastalarının KVH risk
faktörleri bilgi düzeylerinin tespit edilmesi hedeflenmiştir. Gereç ve Yöntemler:
Atatürk Üniversitesi Romatoloji Polikliniğine başvuran hastalardan
“Assesment in Spondyloarthritis International Society” sınıflama kriterlerini
karşılayan 18-60 yaşları arasında AS’li ardışık 101 hasta çalışmaya dâhil
edilmiştir. Katılımcıların demografik verileri ve klinik özellikleri kaydedilmiştir.
AS Yaşam Kalitesi Anketi ile yaşam kaliteleri belirlenmiştir. KVH
risk faktörleri bilgi düzeyleri, Kardiyovasküler Hastalıklar Risk Faktörleri
Bilgi Düzeyi (KARRIF-BD) Ölçeği ile ölçülmüştür. Bulgular: Katılımcıların
KARRIF-BD Ölçek skoru ortanca değeri 18,0 (minimum-maksimum:
2-26) hesaplanmıştır. KARRIF-BD skorları; 35 yaş üstü grupta, hastalık süresi
10 yıl üstü olanlarda, ek hastalığı bulunanlarda, KVH olanlarda, ara sıra
egzersiz yapanlarda daha yüksek tespit edilmiştir. Diğer parametreler arasında
korelasyon veya fark saptanmamıştır. Sonuç: AS hastalarının KVH
risk seviyesi, genel toplumun üstünde olmasına rağmen bu hasta grubunun
KVH bilgi düzeyleri beklendik oranda yüksek değildir. Bu çalışmada, AS
hastaları için KVH bilgilendirme programlarının gerekliliği ortaya konmuştur.

ABSTRACT
Objective: Cardiovascular diseases (CVD); defines a group
including stroke, peripheral vascular diseases and coronary artery diseases.
Ankylosing spondylitis (AS) patients have an increased risk for CVD. Mortality
rates of AS patients are 1.5 times that of the general population. Cardiovascular
events are responsible for 20-40% of mortality in these patients.
Various tools and methods are being researched to detect and reduce this
risk. In this study; in order to be a basis for researches, it was aimed to determine
the CVD risk factors knowledge levels of AS patients. Material
and Methods: A total of 101 consecutive patients with AS, aged 18-60
years, who met the Assessment in Spondyloarthritis International Society
classification criteria, were included in the study from the patients who applied
to Atatürk University Rheumatology outpatient clinic. Demographic
data and clinical characteristics of the participants were recorded. Quality of
life was determined by the Assesment in Spondyloarthritis International Society.
CVD risk factors knowledge levels were evaluated with the Cardiovascular
Diseases Risk Factors Knowledge Level (CARRIF-KL) Scale.
Results: The median value of the participants’ CARRIF-KL Scale score
was calculated as 18.0 (minimum-maximum: 2-26). CARRIF-KL scores; it
was found to be higher in the group over the age of 35, those with a disease
duration of more than 10 years, those with additional disease, those with
CVD, and those who exercise occasionally. No correlation or difference was
found between other parameters. Conclusion: Although the CVD risk level
of AS patients is higher than the general population, the CVD knowledge
level of this patient group is not as high as expected. In this study, the necessity
of CVD information programs for AS patients was revealed.
