Ailevi Akdeniz Ateşinde Cinsiyetin Depresyon, Anksiyete ve Yaşam Kalitesine Etkisi

ÖZET
Amaç: AAA (Ailevi Akdeniz Ateşi) hastalarında diğer kronik hastalıklarda olduğu gibi depresyon,anksiyete ve uyku bozukluğu sıklığı artmıştır. Ancak cinsiyetin depresyon, anksiyete ve yaşam kalitesine olan etkisi net olarak araştırılmamıştır. Bu çalışmanın amacı; AAA hastalarında cinsiyetin, hastalık klinik özellikleri ve hastalık aktivitesi ve Hospital Anxiety and Depression Scale (HADS) ve Nottingham Health Profile (NHP) kullanılarak anksiyete, depresyon ve yaşam kalitesi parametreleri üzerine etkisini incelemektir. Gereç ve Yöntemler: Çalışmamıza dahil edilen AAA hastaların hastalık başlama yaşı, hastalık süresi,1 yıldaki atak sayısı, atak özellikleri, amiloidoz varlığı, MEFV gen mutasyonu sonucu, AAA için aldıkları medikal tedaviler ve International Severity Scoring System (ISSF) not edildi. Hastaların ağrı ve yorgunluk düzeyleri visüel analog skala (VASağrı/ VAS-yorgunluk) ile ölçüldü. HAD-anksiyete ve HAD- depresyon skalası, AAA hastalarının anksiyete ve depresyon düzeyini ve şiddetini, NHP ölçeği yaşam kalitesini değerlendirmek için kullanıldı. Bulgular: Hastalar cinsiyetine göre iki gruba ayrıldığında, yaş ortalaması, VKİ, medeni durumu, eğitim durumu ve sigara kullanımı açısından aralarında istatistiksel olarak anlamlı farklılık yoktu (sırasıyla p=0.218, p=0.383, p=0.505, p=0.246 ve p=0.411). Hastaların cinsiyetine göre HAD-depresyon ve HAD-anksiyete skorları karşılaştırıldığında kadın hastaların erkek hastalara göre depresyon ve anksiyete skorları istatistiksel olarak anlamlı bulundu (sırasıyla p=0.01, p=0.04). Kadın AAA hastalarında erkek hastalara göre NHP uyku ve emosyonel reaksiyon skorları istatistiksel olarak anlamlı ölçüde daha yüksek bulundu (sırasıyla p=0.027, p=0.033). Sonuç: Çalışmamızda, her iki cinsiyette benzer hastalık aktivitesi durumunda dahi; kadın AAA hastalarında erkeklere göre daha çok depresyon, anksiyete, uyku bozukluğu ve sosyal izolasyon gözlenmiştir (sırasıyla p=0.01, p=0.04, p = 0.027 ve 0.033). Bu nedenle hastalığın bu açıdan değerlendirilmesi yararlı olacaktır.
ABSTRACT
Objective: The aim of this study is to examine the effect of gender on the parameters of anxiety, depression and quality of life in FMF patients using the disease clinical features and disease activity and the Hospital Anxiety and Depression Scale (HADS) and the Nothingam Health Profile (NHP). Material and Methods: Age of onset of disease, duration of disease, number of attacks per year, attack characteristics, presence of amyloidosis, MEFV gene mutation result and the International Severity Scoring System (ISSF) were noted. HAD-anxiety and HAD-depression scales were used to evaluate the level and severity of anxiety and depression and the NHP scale was used to evaluate quality of life. Results: When the patients were divided into two groups according to their gender, there was no statistically significant difference between them in terms of mean age, marital status, educational status, and smoking (p=0.218, p=0.505, p=0.246, and p=0.411, respectively). When the HAD-depression and HAD-anxiety scores of patients were compared by gender, the depression and anxiety scores of female patients were found to be statistically significant when compared to male patients (p = 0.01 and p = 0.04, respectively). NHP sleep and emotional reaction scores were found to be statistically significantly higher in female patients compared to male patients (p = 0.027 and 0.033, respectively). Conclusion: In our study, it was revealed that female FMF patients had more depression, anxiety, sleep disorders, and social isolation than males (p=0.01, p=0.04, p = 0.027 and 0.033, respectively).
